Position Description

Lighthouse RN Case Managers work with a variety of patients on hospice care (in their home or in a contracted facility) and provide direct nursing care along with a variety of other health-related support tasks.
Responsibilities:
● Monitor and record patient's condition
● Document and chart patient's vitals
● Adhere to caregiving regulations and standards
● Thrive in and cooperate well with a multidisciplinary team

Required Experience

RN Case Managers are required to have a diploma from a Nursing School and a valid SC RN license. Nursing experience required, hospice experience preferred.
