Position Description

Care For Life is seeking a part time RN familiar with senior related care, issues and challenges to join our Care Management team. Along with community outreach and education, Care Manager will have an opportunity to work proactively with our senior clients. Care Manager will assist in identifying needs and corresponding resources to create aging life plan based on individual needs and preferences of client. Care Manager will establish and cultivate an on going relationship with clients, family members, physicians and other providers.

Position has the potential to grow to full time.

Required Experience

The right Candidate will be respectful of clients right to self determination and knowledgeable of the challenges faced in the Aging journey.

Must have valid nursing license in good standing along with clean driving record to be considered for the position
