LONG TERM CARE OMBUDSMAN

GENERAL PURPOSE
Implements the regional provision of the Long Term Care Ombudsman Services.
Reports to the Aging Services Director and the Regional Long Term Care
Ombudsman.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsible for advocating for the rights of residents and better quality of care and
quality of life in long term care facilities in accordance with state and federal laws.
Receives, investigates and resolves complaints regarding long term care facilities
with priority given to facilities operated or contracted for operation by the SC
Department of Disabilities and Special Needs or the SC Department of Mental Health.
Prepare and execute outreach presentations and informational trainings on various
residents’ rights and long term care issues.
Implements states’ and regional policies and procedures for the Long Term Care
Ombudsman program.
Conducts routine facility visits on behalf of the LTC Ombudsman Program to
advocate for the resident, make observations, provide information and resolve
problems on behalf of the resident.
Maintains records and accurately completes required reports and documentation of
case-related information. Upholds confidentiality of client information in
accordance with the Older Americans Act. Conducts research and develops
background materials, reports, etc. as needed on advocacy issues.
Promotes public understanding of residents rights, and other long term care matters
including development and distribution of materials, public speaking and training
activities.
Provides ongoing programmatic training and consultation to families, resident
councils and staff of long term care facilities. Promotes community and volunteer
support for residents of LTC facilities.
Attends professional continuing education, conferences, and training as permitted

DESIRED MINIMUM QUALIFICATIONS
Education and Experience:
(A) Graduation from a four-year college or university with a Bachelor’s degree
in social services or a closely related field;
(B) Two or more years of experience in human services or long term care
services preferred; or
(C) An equivalent combination of education and experience.
SPECIAL REQUIREMENTS
 Valid state driver’s license or ability to obtain one.
 Successful completion of activities that lead to certification as a Long Term
Care Ombudsman. These include completion of State Long Term Care
Ombudsman Program facilitated training program and a probationary
complaint investigative period under direct supervision of Regional Long
Term Care Ombudsman.
 Daily travel is required; some overnight travel may be necessary.
 Investigations and routine visits may require after-hours and weekend work
