Please join the early/mid-stage dementia activity group and a guest for this afternoon of fellowship and creating your very own take-home planter and plant some Forget-Me-Not flowers.

The Alzheimer's Association Connections group will provide the Forget-Me-Not seeds and planters...The Pearl at Five Forks will provide the soil and light refreshments...Must RSVP by 5/20; however, supplies are limited and the spots will be offered to the first respondents.

THIS ACTIVITY IS NOT INTENDED FOR PEOPLE LIVING IN A LONG-TERM CARE FACILITY.
